Despite being careful I found I was missing second off sometimes still and it got me to thinking: you don't shift into first and teh clutch isn't activated until the first time you press it so maybe there is some kind of delay the first time it's used and since that delay occurs between first and second, that's why so many second miss shifts.

So during the countdown I pressed the clutch in once to activate it and I feel now it works better for shifting into second off the line.

It might be my imagination but thought I would mention....

I think it has something to do with the RPM being too high to enter into second gear. It seems to happens worst for some cars (especially older models). I've also missed the gear with heel and toe when down shifting by giving it too much gas before entering the gear.
p.S this maybe a counter to the clutch cheat that was in GT5P. I don't really know how that cheat worked though.

That happens to me ALL the time. That's why I use sequential until I reach 3rd gear before I activate the clutch. Annoying as hell.

Maybe second gear is just a too low of a gear for the car to handle. I'm not sure about GT5 or Prologue or the G25 or G27 wheels, but if I know anything from real life is that trying to go into gear 1 or 2 at higher speeds, the shifter is either hard to go in or doesn't even go in to gear one. Maybe you should try going to three first and see if it goes in fine then and your RPM is too low, stick it into second.

Second is notoriously sticky off the line as well as downshifting so it's not necessarily a case of too fast for the gear :(

I seem to be missing second ALOT right at the start of a race too. I never thought of it like this though. I've just been a bit more careful and don't slam it.

Shift slowly into second gear when acclerating so the rev,s can drop a little more and it will shift fine. Alot of cars have a big gap between first and second gear first is a real low gear for hill starts. You will also find if you upgrade the tranny to a close ratio 5/6 speed it will work better

PD needs to fix this...it's unacceptable:

a) to have an on-off clutch while our systems support analog clutch,

b) not being able to shift while at high rpms.

Just let off the accelerator before shifting from 1st to 2nd and it will work everytime. I forgot about this issue that was also in GT5P. You just have to re-train yourself but you won't lose that much time after you get it down.
