A funny quest...

I heard many conversations about sore, itching thumbs. Now, I experienced the same pains as well, and finally found a button setup which, although harder to drive with, solves the issue for right-handers.
Steering- right stick
Accel.-R1
Brake-L1
Reverse-left stick down
Handbrake-none
Gear up- dir. button up
Gear down- dir. button down
Rear view- R2
Change view-SELECT
Special-L2
Ghost-X

The problem is that the R and L buttons (R1, R2, L1, L2) aren't pressure sensitive as X, square, triangle and circle are, so whenever you accelerate or brake, you''re essentially flooring it. Sucks for slow corners and uphills, unless you tap the buttons repeatedly, but that certainly takes out the alleged reality of it.
 
The R and L buttons are just as pressure sensitive. I use R1 as the brake, and I'm freely able to modulate the braking strength.

This set up would drive me absolutely crazy. But whatever works for you...
 
Just tried it out, and of course it would take a bit of getting used to, I'd like it. The R1 + L1 just don't have enough accuracy as the Analogs.
 
This would probably work great for me since I'm using an old third-party controller. Like Korza said, I use the right stick for accel and brake usually, but my left thumb gets bothered after a while of playing.
 
Thanks to my ps2 my thumb has a permenant indent in it from the x button. But back then I couldn't drive it in any other combination it just didn't feel right.
