Gt4 Error!!! Help Needed!

Hi guys, I've looked everywhere but I dont couldn't find anywhere for the solution of my GT4 error. Recently everytime I play GT4, I can enter a race and after a race there is this red revolution thing just going round and round.

Would load the next scene, So... I was wandering if anyone had experienced this problem and know how to fix it. Please let me know?!
I tried restarting, I even deleted everything and started all over and this happens again! So if you know the solution, you will be my GT4 saver!
 
What is the surface of the disk like?
Is the PS2 you have the old Big one or the new Slim one?
 
DVD Problem
try another DVD or another Consol
 
Yeah that happened to me too, but two or three times. It's 95% the DVD
 
[search]Disk Read Error[/search].
