Found it, finally. It's "Spider" - 2002, by David Cronenberg, with Ralph Fiennes. It can be watched on Internet Archive.

a man gets off a train in a large station in the UK, maybe London but I'm not sure, he's wearing a coat and carrying an old suitcase. He takes out a crumpled piece of paper with an address written on it where he has to go (a small asylum for the mentally ill, a woman is in charge of it). You can tell that the man, the main character, is clumsy and has mental health issues, he doesn't speak, and writes his thoughts in a dense script on little notes. Other scenes I remember include flashbacks where the character relives scenes of the difficult relationship between his mother and father, maybe a plumber. The father has dealings with prostitutes and will eventually murder his wife, (maybe again) burying her in a garden.
