T-GT2 wheel keeps coming out

Hey guys, been playing GT lately and went to start it up last night and my wheel literally stopped working. I fixed this issue just now but noticed that my wheel keeps coming out. It's not the wheel itself but it's the collar that goes over the driveshaft.

Think the photos have been uploaded... Would anyone know as to why the collar for the driveshaft keeps coming out (making the whole wheel come off)?
 
Perhaps if you remove the quick release from the rim it will help you understand the problem.

For some reason I can't see those other pictures from my work laptop. So ignore that message.
