That would be Nine Inch Nails' Broken. One of the simplest and best-executed graphic design packages of all time. Especially in conjunction with the remix album, Fixed:

The one I used to have opened in 3 "flaps" to reveal the CD... I always thought that was one of the most creative CD cases ever.

Yeah, and the reversed (black background, flaming letter), backwards 'n' from your second picture was the inside of the left flap, the 'i' was down the meridian of the CD itself in the middle, and the reversed, frontwards 'n' was on the inside of the right flap, so when it was open, it made the lowercase NIN logo. Then Fixed was identical, except in the cool blue, not the hot orange.

Living proof that less is more.

Yes it is. I think Animate, the first song of that album is one of my all-time favourite Rush songs.
