I think the only difference is that the image's full size are different. First photo is bigger than the second.

What square headlights? Do you mean headlights of the old ITR? I guess so..

The Integra Type R 98 Spec. has change his wheels than the 95 spec., the gear ratio are different too. Some changes about the suspension too. For the 99 spec, i don't know what's the difference... There's the yellow color.

"95" Specs are the first model, with 4 lug wheels. The '98 and '99 Specs were the last run models, a bit of an evolution, if you want. They have 5 lug wheels, 200hp, revised suspensions and a bit less weight. The real cars had HID's as well.

Should be in GT Auto > Wheels > At the very bottom. At least, that's where it is on the Japanese version.
