The only real problem with waitresses (usually the men are better at this) I have is snottyness. I have been to some high class resturants (like suit and tie) where I have had food cold, where I asked for a warming.
I didn't want a new plate, just to have the food warmed up a bit (I think it was steak and something else) and the waitress gave me a "who me?!?!" look, and consiquently gave me bad service for the rest of the meal. I in no way acted out of normal, and politely asked to have it warmed.
The only other thing is the way customers act toward the waitresses, also, this usually dosn't happen with male waitors. It seems that many customers will mess up their own order. They will order something, and then say that they didn't order things and such. And getting mad at a waiter who has no controll over food prep, about overcooked steak... at least be civil about it.
