I also called and am getting the form emailed over.
 
The form is being emailed over to me also. Just call the number at the top left of the form. I think enough of us have called by now as soon as we start talking they're like "hold on whats your fax or email and we will send the second page right over". LOL.
 
Has anyone gotten the form via email yet? Still hasn't arrived here. Brian, can you email it to me if you've gotten it already?
 
If any of you need the forms list your emails and I'll send them your way. I received two additional ones to the one that came in my original email.

Apparently the doctor didn't give it to Robin. She's going to spam out the backside once she gets it.

I was a part of both the 2011 & 2012 GT Academy National Finals and I have to say this is by far the most unorganized backwards pre national finals preparation yet. Between the late notices and the wrong documents being sent is has just been a hassle.
 
That was fun, I think the lady who finally ended up doing mine thought I was trying to scam them or something. She asked me to explain what was going on, and who sent me the email. She laughs and throws the form back at me saying she can't do it, looking at me like I'm crazy.

An hour later she's poking me with a needle.
 
Out of curiosity was there a delay from when you sent in the first notarized form and copy of your ID till you got more things to do? I haven't got the request to get blood drawn yet which I'm just guessing is because I was told I'm in a few days ago. I'm finding it hard to just wait for more info but don't want to be a pest.

Good thing I'm just happy to go. It could be in a dusty basement run by trained mice for all I care.
