Just to double check are you sure it installed on the PS3. If so, is it not showing up in the Main Menu at the Intro screen? If not try to reinstall it again.

DLC region matches store region matches account region. In order for the DLC to work it must match the disc region too. If you've downloaded it from an Asian store for a US disc, it won't work.

Can't remember if it works for this game as I haven't been able to get my PS3 online for a while but couldn't you set up another PSN account as American and download it from that store and then activate it on both accounts?
I might be completely wrong but I'm sure there's a workaround for this.
For this, yes. He has a US disc, so he needs a US download from the US PSN store which means a US PSN account. Once it's installed in the US save file on that US account it'll be available to install on any game save on the console.
