No, not utopia. This album is from a side-project of a Dutch guitar player and it counts with many guests. There was a live album released afterwards.

You can pretty much figure the project name by the logo appearing on the cover.

That's definitely Manowar and the album is called "Kings of Metal" :)

Nope. (Manowar's covers have to do always with their mascot or with a battle theme, that's how I knew Samurai's cover ;) )

Clue: They are a Finnish power/heavy metal band and were formed in 1984. They are quite famous. This album was released in 1994.
