What does it mean? It says to complete 10 melee counters in 4 minutes. Does that mean beat 10 people to death with your bare hands in 4 mins?

No - you need to perform a Melee counter.

It tells you how to do this in a mission early on where you have to beat up two guys who are laying into Roman. Essentially you have to wait for a punch to come in, block it and smack them back. It's quite tricky to do - though you'll get the hang of it quickly. You'll also need at least 3 victims and a quiet place which sees plenty of people but no cops.

I tried passing that with a baseball bat & Knife-- Melee weapons... before reading actually how to do it...

What are the controls for countering?? I don't recall--(I normally counter with bullets).... doesn't seem to show it in the pause menu??

I read a good place for doing this is the heli tours landing area-- if you get low on health, you can get food at the vendor out front.

//Edit// Nevermind, I figured it out-- one thing I found though-- I was doing this at the heli tours area and got killed, I respawned at the hospital, immediately got in a taxi and went back and resumed... and got the trophy immediately after resuming (first counter)-- so evidently dying does not reset the 4 minute clock.
