PSU problems

I have been having a non stop intermittent issue with my system randomly shutting down.
The message I get talks about a surge occurring.

So for the past week I have been logging the voltages from the computer.
And at times after the crash and I look at the log I see the +5v rail showing 0.000v in the log.

This happens during gaming, during video encoding, during web browsing, while watching TV on my PC

So it is random and not caused by a single thing.

My PSU is a OCZ 850watt zx

I read nothing but good things about it that is why I got it.
Has anyone else has similar issues from a PC PSU?
 
Could be the surge board causing issues. Why not try plugging the PSU directly into the wall and check the voltages then? It probably isn't the surge protector but it's worth a shot I guess.
 
Not the surge board since only the +5v. +5vsb and +3.3v rails are affected.

Everything else is normal.
 
This sort of thing is not from a surgeboard.

I have other things connected and they are fine.
The surge board just filters mains voltage and it is a $200 filter which is not cheap.
 
How long have you had the PSU?

Only thing I can think of is checking your USB devices. USB & Drives are the main components that run on the 5v.

I've seen before on older computers were too many USB devices can plague a computer on start up.

Any bulging capacitors on your motherboard?
