1. I average about 1.5 cups a day.
2. Nothing fancy, but I do like it strong. I was a Maxwell House fan for about 8 years, made a switch to Folgers recently. Fancy stuff is alright, but not worth it on a daily basis. My brand of choice is the Folgers Colombian (the darkest). If I can see the sides of the cup/mug, that is not coffee in my book.
Usually a half-packet of sugar, unless it's flavored, then I don't use any. The only exception is "coffee on the fly that's two days old"...add three packets of sugar. I add cold cream (not powder) if it's a hot day; I hate sweating after a cup on a 90-degree morning with 99% humidity outside.
3. I was on a cruise ship with my buddies a week before senior prom; we drank (we weren't big drinkers) beers all night, and were ready to pass out during the buffet. They were getting coffee for desert, and i thought I'd try it. Needless to say, I couldn't sleep all night.
4. Yes, addicted. I need that second cup somedays; unless I'm very well rested the night before, I need it about 2 hours after breakfast if I don't have it. Funny, I don't miss it it when I'm sick, I sub tea in place of joe. I've tried to quit, but I replace it it with twice the tea, so I don't think that's really the way to go. That said, I promised to quit coffee for a time when Kathy and I try to have kids.
5. It's all rather the same to me, nowadays. I was spoiled at my last job: Aramark had a coffee bar concept called Java City. There was literally 50 ways to enjoy coffee beans. I was spoiled for a while there; very nice to have a place to work that made you whatever you wanted...expresso, cappuccino, mochas, fancy teas, etc. for free!
