My headcanon explanation:

Nightmare Moon is an evil parasite that latches onto ponies' consciousnesses by exploiting their weaknesses, (in Luna's case, her loneliness) and takes over their minds and bodies by slowly corrupting them.

Most likely, it was weakened by the Elements of Harmony first time around, but was not removed altogether - and some as-yet unexplained event in S4 has triggered it again.

Makes sense to me.
