I remember the gorilla mask, but I couldn't remember the ghost mask. I though it was something other than that. For a minute I though it was "Tricky Dick."
 
Didn't he have a sheet over him with eyeholes cut in it? And when they unfroze Michelle, after the dramatic reveal, didn't he go "wooOOoooo" in a ghostly style right before the commercial break? I believe so...



EDIT: The answer you're probably looking for is gorilla and giant fly, but he was also a ghost. I checked last night.

During Futurama's entire run, Fry loses several parts of his anatomy - some regained, some replaced. What are they - and how?

Answers:
Head - in a hovercar crash with Amy. It's grafted onto Amy's body and reattached later in the episode, once his body has recovered (Put Your Head on My Shoulder)...
Hands - Temporarily swapped with the robot devil's hands (The Devil's Hands Are Idle Playthings).
Hands again - Eaten by a T. rex, and replaced at the Handcrafters. Commented that he'd be breaking them in that evening... (I Dated a Robot).
Right Arm - Zoidberg chops it off in a duel, while Fry is making an important point about friendship being more important than love (Why Must I Be A Crustacean In Love?)...
Teeth - Fry loses several to Slurm (Fry and the Slurm Factory).
Nose - Removed when abducted by aliens, to use as an aphrodisiac called "Human Horn". And comes within milliseconds of losing his "lower horn" too... (Spanish Fry)
 
He loses his nose, or "Human Horn" in the episode entitled "Spanish Fry", after being abducted by aliens on a camping trip. It is later bought by Lrrrr. Lrrrr and Ndnd believe it to be an aphrodisiac, and he nearly loses his lower horn, after Bender alerts the Omicron Persei 8 leaders that his nose is not his productive organ.

He also loses his hands for a while in the final episode, "The Devils hands Are Idle Playthings" where his hands are exchamged for the devils in a highly ironic turn of events.
 
Fry loses 10 teeth when he visits the Slurm factory, I think...

Edit: Oooh, ooh, Fry loses his whole body in a hovercar crash and his head is grafted onto Amy's body. Does that count?
